Dental Insurance
Your insurance is a contract between your employer or university and an insurance company. The benefits received are based on the terms of the contract negotiated between your employer or school and the insurance company.
Our office will do everything possible to help you understand and make the most of your dental insurance benefits. Prior to any scheduled procedure, we will provide you with a form to submit to your insurance company; this will allow you to know in advance what portion of the cost will be covered. At the end of each treatment, we will provide you with another claim form to collect the appropriate amount from your insurance company. Please remember that you are ultimately responsible for all costs incurred. We urge you to read your insurance policy to be fully aware of the coverage and any limitations of benefits provided.
